Evaluations Including DUI or DWI Evaluations

Jan Williams will do evaluations online by email or by Skype, or in person. Call 443-610-3569 for information about office location (Forest Hill, Maryland). State laws vary in regard to DUI evaluations and DUI education. Please get approval from the court, DMV or your probation agent, or your attorney, before making a payment for Jan Williams' services. Jan Williams is a Maryland Licensed Clinical Alcohol and Drug Counselor (Maryland Department of Health and Mental Hygiene, Maryland Board of Professional Counselors and Therapists (LCA #280)).

As soon as Jan Williams receives notice of payment, if this is an online evaluation, he will send you an email with an attachment containing a packet of forms to fill out and return providing him with information about your use history that will form the basis for the evaluation. In the case of an in person evaluation Jan Williams will, after he receives notice of payment, call you and schedule an appointment to do the evaluation. After getting the data for the evaluation, Jan Williams will draft a letter to be sent to the appropriate individual with conclusions about the presence, or not, of a problem with alcohol and recommendations, or not, for treatment and/or education. You will have the opprotunity to approve the letter before it is finalized and sent.

For a DUI or DWI, you will need to provide the following information by FAX or electronically through an e-mail:

• Court Papers (Summary suspension, Court Order)
• Drivers Abstract (copy of driving record from DMV)
• Copy of Photo ID or Drivers License
• Printout of All Previous Offenses - if any
• BAC (if not refused)

Check with your court, DMV, probation officer, or lawyer to be sure a Maryland licensed addictions counselor's evaluation is acceptable.
